Teacher Licensure or Preparatory Course Certification Appeal Form
The Teacher Licensure or Preparatory Course Certification Appeal Form is mandatory for students seeking aid eligibility who have been admitted to the University of Tennessee, Knoxville, and either:
- Need to take pre-requisite courses as a non-degree seeking student in order to gain admission into their specific degree program, or
- Are taking courses toward a teacher’s licensure.
The Teacher Licensure or Preparatory Course Certification Appeal Form is NOT for students who are taking courses at the University of Tennessee, Knoxville that they plan to transfer to their home institution. If this is your situation, you must complete the Consortium Agreement form.
Instructions for Accessing the Teacher Licensure or Preparatory Course Certification Appeal Form
- Log in to MyUTK using your NetID and password
- Select TN HOPE, SAP & Requirements from the My Financial Information section
- Select Manage Requests
- Select Teacher Licensure or Non-Degree Preparatory Course Certification Appeal
- Follow the on-screen prompts to electronically sign the form
- Use the download link provided to save a PDF of the form
Instructions for Completing the Teacher Licensure or Preparatory Course Certification Appeal Form
Once you electronically sign the form, you will be provided with a download link that generates a PDF copy of the form. Once you have downloaded the form, you must take it to your college’s Dean for an ink signature.
Instructions for Submitting the Teacher Licensure or Preparatory Course Certification Appeal Form
- Log in to MyUTK using your NetID and password
- Select TN HOPE, SAP & Requirements from the My Financial Information section
- Select the Needs Action section from the dashboard
- Select the Upload option for the Teacher Licensure or Preparatory Course Certification Appeal Form
- Submit the final version of the form
Drop Full-Time to Part-Time Enrollment
This option is used to request a change from full-time to part-time enrollment in the same semester. Students enrolled in a full-time status, as of the 14th day of classes, may not drop or withdraw to part-time status within the same semester without prior approval. Tennessee Law limits the reasons for enrollment status change to documented medical or personal reasons only. Students must include documentation with their request to be considered for an enrollment status change.
Drop Part-Time to Less than Part-Time
This option is selected to request a change from part-time to less than part-time enrollment in the same semester. Students enrolled in a part-time status, as of the 14th day of classes, may not drop or withdraw to less than part-time status within the same semester without prior approval. Tennessee Law limits the reasons for enrollment status change to documented medical or personal reasons only. Students must include documentation with their request to be considered for an enrollment status change.
Leave of Absence/Co-Op
Students may request a leave of absence for personal (illness or death of an immediate family member), medical, or military mobilization reasons. Students may also request a leave of absence during an internship or co-op program that is required or encouraged as a part of the academic program in which the student is enrolled. Students should include any supporting documentation with their application. Students whose requests are approved will be granted a leave of absence without affecting their eligibility to retain the TELS Program Scholarship.
Eligibility Appeal/Non-Continuous Enrollment/ Withdrawal
Any student who wishes to appeal the denial or revocation of the HOPE Scholarship should select this option on the HOPE and Institutional Scholarship Appeal Request Form. Students should include any supporting documentation with their appeal. An Institutional Review Panel (IRP) will review appeals and the student will be notified of the panel’s decision in writing. If the IRP denies your appeal, you have 45 calendar days to appeal, in writing, directly to the Tennessee Student Assistance Corporation (TSAC) TELS Award Appeal Panel. You are not permitted to appeal directly to the TSAC TELS Award Appeal Panel unless the UT IRP denies your appeal.
Review of HOPE GPA
Select this option to request a review of your HOPE GPA to determine your eligibility to regain the HOPE Scholarship. You may only regain the HOPE Scholarship one time during your college career.
Notification of Repeated Class Grade Replacement
Use this online form to notify the Office of Financial Aid & Scholarships of your intention to use your one-time course repeat option. This option utilizes the highest grade for a repeated course in the TELS GPA calculation. However, the semester hours for both attempted courses will be included in the total HOPE attempted hours. This option can be used to either retain or regain your HOPE Scholarship.
